WHAT DO I NEED FOR LIVE SCAN?

Bring a completed “Request for Live Scan Service”, a 3-part applicant submission form. This form must be filled out with information provided in all required fields, including your Social Security Number.

As a courtesy to our clients we have blank “Request Live Scan Service Forms” available for your convenience. It is your responsibility to obtain the necessary ORI Code.
